I recently re-installed Civ after a few months hiatus. I reinstalled vanilla civ and then PTW on top of it, but only installed the PTW 1.21 patch. Do I have to also patch vanilla civ with 1.29 or is that included if i patch up to PTW 1.21?

PTW will patch Civ3 to v1.29f automatically. You don't need to worry about it once you've installed PTW. ;)
