I believe they have to be on their own individual islands to get the resource.

The 3 cities have to be in a different island than the island your capital is in. Once you have the 3 cities up in other islands, your 3 cities become unrazable.

No, they must be on separate islands.
Also, the city doesn't have to be originally settled by you. It works for AI cities (not City states) that you gain control over.
For example, I had a game where another AI city on another continent flipped to me due to tourism pressure. I got the bonus luxuries for the city that flipped.

I guess it could be easy to ban indonesian resources since there are only 2 copies. Unless gajah takes all the world Congress votes.

What if you weren't playing as Indonesia but gifted an island city to Indonesia, would that become a spice island city? If so would it remain as one when you took it back through force or other means?

No, a city gifted to Indonesia wouldn't become a spice city. Indonesia has to build the spice city in another island with a settler to get the 2 luxuries.
