I looked at save. and all the northeastern cities that is linked to the capital by rails has the bonus (all eastern cities up to and including Kufah ,excluding Stockholm)
There is a wonder or social policy that adds a hammer to specialists.
Maybe you've overlooked the minus.
Something lowers the city production in Shanghai by 1 from a specialist, but the city isn't running any specialists.
It's the only city that has this problem and it's also the only city that has a workshop.
Oh, I misunderstood. I will try out the save.

Just looking at the moment. I can indeed see the -1 from specialists.
Also, did you notice that the graphics for the city of Antium look really bare? More sparse than the buildings in a trading post.
By the way, I can confirm that I can't see anything wrong with which cities are/aren't giving the +50% hammer bonus from railroads. The rail network is pretty patchy, some parts are only ordinary road.
