I was playing as Carthage, patch 1.22, Archipelage, Larger map, 12 civs, and I placed a city, I went back and it had no name.
I can only think of one reason for this. When you place a city and it asks you for the name, I'm pretty sure it highlights it, meaning that if you accidentally hit backspace then the name will be deleted. I may have accidentally done this then pressed enter, but I'm not so sure.
You can see it everywhere. In all the advisor screens. When you try the locate city feature (ctrl-l) then if you press one square below the bottom city and double-click it, it goes to that city. One time the Military Advisor even said "There are barbarian tribes near !"
Here's the save. It's the city in the south on the flood plains.
