This is an interesting one, but I think I've narrowed down the problem.
If two cities can access an improved iron resource, you won't be able to build a forge in either city. My particular scenario has one city seeing the option to build a forge, but it being greyed out with the "must have a source of improved iron nearby", and the other doesn't even have the option. Interestingly enough, it's the first city built near this resource that does not even have the option (no, there is no forge in this city, I checked for my own stupidity.
). The second (and closer to the resource) city has the option greyed out, even though it's already working the improved resource.
I've tried swapping ownership of the resource between the cities in hopes of freeing it up, but it's not working.
Iron resources "owned" by only one city do allow forges to be built properly.
