The Fog of War refers to the fact that you can only see stuff where you have eyes to see it.
So you can see the units near your units and cities, but you have no idea what is going on behind the border.

1. in game= the invisible, black areas of the map. where you havent explored yet.

2. in editor= the fog of war is already set to be on. that option is actually getting rid of the fog of war, by "painting" the tiles with it selected.

Usually, the term fog of war means the gray area that you have explored but can't see ATM. But in the editor, removing the FoW from a tile reveals makes it explored for everyone, but does not give line of sight.

In a general sense (not just Civ) Fog Of War means the things that happen that you can not see. It would certainly mean "the inky blackness" (a friends phrase for unexplored areas), but it also would apply to areas explored, but not currently viewed. The phrase greatly predates any version of CIV.
For CIV it seems to mean just the inky blackness.

well, not just the inky blackness - it's anything that units aren't looking at. You may have explored a territory, but if you dont' have anyone there and run an unescorted settler by it, you can find barbarians or German archers there... not that either of those things ever happens to me, of course...
