When someone is banned they will get a dialogue box that tells them that they have been banned, the reason, and for how long.

Yes.

But what about in the post that left them a ban? Or in other words, can you be banned randomly, well not randomly, but the moderators decide you've flamed, etc. too much, and be banned?

When I've been banned, it has never been at the moment of the "alledged" transgression. I come back later and discover it.

Sometimes if an example needs to be made a ban will be posted about. other times it is just 'done' without any forum notification in posts.

Is there any significance to the colour of the Mod tags used? Or is it just personal preference? I've been vaguely assuming it's red for warnings, blue for not-quite-warnings and green for actions like thread-merge etc. (Please don't smite me for PDOMA - I'm just curious.)

A lot is personal preference, but I think, in general terms, you've more or less hit on the usage/meaning.
