One World Expansion suggestion - civil wars

I like the idea of a loyalty mechanic. newly conquered cities would have a lower loyalty rating naturally, but it could rise slowly over time. puppet cities would be stuck at a lower threshold until annexed, and then you couldnt get to the higher loyalty settings without a courthouse for example. With that train of thought, you'd have a trade off between having loyal cities or having a civic poor empire since annex cities make civics harder to acquire.
There should be +loyality and -loyality, and like religion if you have many cities with -loyality cause of conqer around own settled city it should get influance from all that -loyality.

That's a good idea. +/- loyalty could spread in a similar way to religion. But loyalty should only be influenced by cities in that one particular civ. If a neighboring civ has an unloyal city right next to yours, it should not affect the loyalty of your city.
