Screen Looks Like 3D

Game worked fine forever until today after I installed windows 8.1. I am using DirectX 10 & 11 to launch game but now my opening is all fuzzy looks like a 3d rendering. I also got a message Civilization V Rating 3D Vision Ready Press Ctrl + Alt + Insert to toggle info. Any help would be appreciated.

You can put on 3D glasses or do what it says for information on how to toggle 3D mode off and on. It would be in the graphics driver, not the game itself.

CTRL+T will turn it off. I'm still not sure how to turn it off permanently though.
