I just started playing Civilization IV a week ago. It seems the game requires smaller maps than Civilization III, so I decided to not try to make another map of Middle Earth based upon the Iron Crown Middle Earth Role Playing system map. I have made a sort of Pangea map with a continent where the west coast resembles Tolkien's Middle Earth, and an island continent looking very much like Numenor lies off the west coast of this super continent. The east coast looks like C. S. Lewis' Narnia.
Now, I'm using a Macintosh, and I'm new to this. What I've done is saved the map in the world builder format, and then zipped it. I've played two games with this map, and it seems to work pretty well. I hope people with Windows will be able to use it as well.
The map is large, so it's about 100 x 60 or so. I made this in Civilization Warlords, so that's probably what you'll need to use it. I had it in the Saves folder in the WorldBuilder folder.
