It's pretty straight forward :

*Victoria is my vassal
*Isabela is my enemy

Isabella had captured a city of Victoria, I reclaim it and the announcer says it has been conquered by Victoria ...
And the control over the city has returned to Victoria, which is strange since it hasn't been asked and my pop-ups are on off.

I had a similar thing happen, although somewhat more bizarre. I was at war with Lincoln and Washington (unrestricted leaders, they weren't allied/vassaled at all), and had a few vassals under me, including Wang Kon. I took a city from Lincoln that previously was owned by Wang Kon - it gave me the option of giving it back to him. I chose to hold on to it. Washington later attacked and captured the city from me. Awhile later, I went back, and re-took it over, but at that point, it automatically gave the city to Wang Kon - didn't ask. As I owned the city at one point, I should have had the option to keep it.

Cities captured from your vassal are automatically liberated to them if you reclaim them. I'm not sure I agree with the mechanic, but that is how it is supposed to work.
