Ambush II and Formation II bugged?

I am currently working on part of my mod that upgrades the Formation promotions to Ambush promotions after Replaceable Parts is researched. This works fine!

Problem: I can't seem to upgrade my units to Ambush II or Formation II. I can only upgrade them to the first level. My mod affects nothing related to this.

I just checked with IGE and found that even in the base game I can't upgrade any of my melee units to Formation II... I was curious if any of you could open up a game with IGE and see if you could not upgrade your units to Formation II as well? Am I missing something here? From the base game XML it seems that Formation II should certainly be an option.

I looked into it more and found that Ambush II and Formation II were missing from UnitPromotions_UnitCombats. I don't know why they were removed.
