Approval Ratings

Can't speak for anyone else, but I'm not sure what you're asking. If you mean the "Approval Rate" or whatever it is in the Demographics window, I wouldn't worry about that, that metric has been full of crap since Civ1. I'm sure there's some mathematical formula, and the code-divers could probably tell you how to maximize it, but I just laugh whenever I see that number.

However, if you're referring to diplomacy, please give us some specifics, because that's a whole big complicated animal.
 
Approval Ratings in the Demographics are nothing to worry about, as they do not affect anything. I have always noticed though that it tends to be higher when there is no rioting and your cities are celebrating "We love the [insert leader type] Day!"
 
Happy / (Happy + Anger)

Which (like Life Expectancy) means that high values are bad - you've probably wasted effort on accumulating happiness or health which you aren't using.

Exactly. There is nothing wrong with bottom approval rating, as long as you don't have angry citizens everywhere.
 
The approval rate is supposed to be the percentage of your own citizens that like the job you're doing as leader. Whether the AIs "approve" of you is a whole different ballgame.
