I enabled the Barbarians back when Civ4 first came out. After several games I went back and disabled it.
If you enable it, it will pop up as a random AI civ sometimes. The AI script for it gives an insanely high chance for it to pop up and ask for things. Getting hit up for stuff every 3-4 turns is darn annoying after a while.
Also, it does gather reaction modifiers, only more slowly than normal. If you tell it "no" enough times, it eventually starts getting mad.
