I'm furious with myself for not having tried this years ago. This method effectively removes the Title bar from the top of Civ4 when running in Borderless Window mode.
Steps: Change .ini entry FullScreen: to "0" and MouseScrolling to 1.
Download ShiftWindow. Installer version worked just fine for me.
Run Civ4 on desired monitor and ShiftWindow in the other. Rename Selected Rule to your preference. Under Window section I selected the Filename box and selected the open Civ4 window from dropdown box. Under Shift To section I selected the Full Screen option and set Target monitor. Setup desired trigger method. Automatic worked for me.
CIv4 will now remain MAXIMIZED when no longer in focus! I run dual monitors and haven't noticed any change in performance or stability yet.
