Maybe they were waiting for the expansion to fix the bugs. The Rise and Fall (of the bugs).

It is due to a mod, check the minimap. More territory is revealed. A mod that reveals more territorry at start is called expanded initial view.

I didn't even think to try when I took that screenshot. However, following a random restart last night I got a similar start:

A question: does that city work like a "mountain canal", so that troops might cross the mountains?

I'm guessing those mountain starts have something to do with a mod as well since I've never seen one in my thousands of starts.

Sadly, it does not. I guess Nepal's ability would involve taunting other players who attempt a domination victory.

What I found amazing about the screenshot was that nobody seems to have been killed off. I usually see 2 or 3 civs die during the ancient age.

That's not really my experience... Typically 2-3 city states are dead by the time the Medieval Era comes around, but that's about it.
