Graphics issues with & without mods

This just started about 2 weeks ago. I've been pouring over the forum trying to find a cure for this, but everything I've tried doesn't work. I would really appreciate anyone's help with this issue. It's driving me crazy. I posted a screen shot froma a game I just started and it is only turn 5. Also posted my DxDiag.
