It can happen with inland cities on larger mod'ed maps as well. This one is ludicrous sized (200x100).

I think it must be a bug with Continents:

Doesn't seem right to me but maybe it's intended.

It was this way in Civ V and in Civ VI since launch. Presumably intended.

But it isn’t limited to Continents maps. Any separate land masses can have this (even if they are on the same Civ VI “continent”)
