Expending is when you use their power or build their tile improvement. Gifting is when you give them to a City-State.

Do CS actually use/settle the GP you give or do they just disappear like the gold you give them?

You gift them like they're a combat unit (through the diplo screen). I don't think they actually get the great person, but I haven't been Sweden since G&K came out and don't remember.
