Not to clear on the title. Is there any way to make improvements make the tile induce damage such as the way deserts deal damage? This could make Forts and a few other newer improvements (minefield, bunkers?) passive but do damage to an attacking force that they'd have to go through.
EDIT: I was fleshing this out so friendly fire does no damage (though I suppose it could be an building option?) via way of upgrade. There is an event where a faction's arrows suddenly are all poisoned; this bonus is applied via special upgrade. Simply provide one that prevents damage done from tiles with friendly fortifications on them and then friendly fire is averted. That might require a unique fortification version for the game for each faction however; I've never modded CivIV myself and I really don't know the details. Also, fortifications could cost maintenance as well as an initial gold cost to build. These shouldn't be spammed, just useful. Perhaps certain units such as spies and recon (and Navy SEALS?) automatically take no damage from these fortifications? Also was thinking somewhat of a naval version, but I haven't fleshed out the details there.
