Luxuries Are Not Being Distributed -- Why?

I have several luxuries (chocolate and coffee) which are only going to 1 city each, even though I have 11 cities total. Every other luxury is going to 4 cities. Why not cocoa and coffee? Which as we all know are the best luxuries in the world!

A few notes:
-- All cities are not Ecstatic, but they are Happy.
-- Cocoa and chocolate hexes do have plantations and are in a city's area of control.

Anyone know what's going on?

Can you post a save file, along with a screenshot of the Resources tab showing how Chocolate and Coffee are being distributed?
