Are you bored of the covid epidemic? Do you pine for a simpler time when hundreds of millions died of a disease that could not be treated? Well you are in luck as we now have Monkeypox to deal with.

The World Health Organization (WHO) chief said on Wednesday that monkeypox infections in non-endemic countries have passed the 1,000 mark and the risk of it becoming established in some is “real”.

Some points we could discuss:

Should we change the name?

Monkeypox does not have much to do with monkeys. It was first identified using laboratory monkeys, but the main reservoir in the wild is rodents: So far, however, only six wild animals trapped in Africa have yielded the virus: three rope squirrels, a Gambian rat, a shrew, and a sooty mangabey monkey. Antibodies to the monkeypox virus are most abundant in African squirrels.

However it is endemic in Africa, and considering the history of the use of terminology relating to Africans this does seem problematic. We have stopped calling covid variants after places, and even excluded a greek letter because of one guy, so it seems there is precedent.

Monkeypox is NOT an STI

The current outbreak has some association with men who have sex with men, and it does require closer contact than covid, but it is not an STI. In the UK all suspected monkeypox cases are being sent to the clap clinic, which is putting them under strain as they are not set up for the this sort of disease, that can be contracted during a consultation. I wonder if it is the governments attempt to stop people panicking by making out it is a problem for those other people that are different from good tory nuclear families.

To vaccinate or not to vaccinate

The only real intervention we have for monkeypox is the smallpox vaccine, the original live vaccine is about 85% effective against contracting the disease. There are new vaccines that are non-replicating and are much safer (but untested vs monkeypox), but the vast majority of the 100 million or so doses of smallpox vaccine that are avialable are still the vaccinia replicating vaccine, and this has serious side effects like encephalitis or progressive vaccinia in about 1 in 25,000 people IIRC. In the current outbreak many contacts of those infected are living with HIV, which could make them more likely to suffer from vaccinia side effects. Given the risks and benefits, “using these vaccines is out of the question”.

Animal reservoirs outside Africa?

In Africa the problem is that it is endemic in rodents, so it cannot be eliminated in humans only. In the 2003 outbreak a number of individuals in the United States caught the virus from infected prairie dogs purchased from pet shops where the animals had been in contact with monkeypox-carrying Gambian pouched rats imported from Ghana. Could we end up with it endemic in American or European rodents?

It seems to me that the covid restrictions should reduce the chance of such spread. Why has it happened now? Could it be a change in the virus? We do not know:

Understanding whether there is a genetic basis for the virus’s unprecedented spread outside Africa will be incredibly difficult, says Elliot Lefkowitz, a computational virologist at the University of Alabama at Birmingham who has studied poxvirus evolution. Researchers are still struggling to determine precisely which genes are responsible for the higher virulence and transmissibility of the Central African strain compared with the West African one more than 17 years after they identified a difference between the two.

It is not considered that airborne spread is a major form. It is found in respiratory secretions, but you need a lot of exposure. Living in dense conditions such as prisons it is a real risk, but you are not likely to catch it in a few minutes in a shop.
Or how it compares to chickenpox?
Clinically it presents similarly (ie. skin eruptions), but it is much more serious.
 
I think the monkeypox right now is an interesting test for preparedness. The alert level is still high, but the disease itself isn't that dangerous or contagious. Given that they've not found out via whom it spreads is a disaster though.

EDIT: I voted GDR, but pretty sure it'll be Covid. Don't ask about in 50 years though.
 
tourist season . There would be no measures anyhow . And yeah , one good thing about being older than 50 would be the vaccine mark on the arm ...

not good precedent. making this connection in the first place says more about the people making it than calling the disease monkeypox. not a fan of the precedent of bending over to china, either.

i would be very surprised if ai algorithms wind up concluding that gdr are the most efficient way to get the job done. unless it has some kind of non-optimizer personality and deliberately makes machines styled after some combination of gundams/voltron/mechwarriors/transformers etc and actually calls them all of those to troll us (even better if it deliberately mislabels them each as other, such that the "gundam" "optimus prime" comes for us). doing this despite the inefficiency. wouldn't be my first guess on the method chosen, but who knows.

It is important that even in these dark times that we maintain hope for a better future, one without war, disease, poverty, or hate.

After all these years, I still believe Giant Death Robots will bring us that future.

WHO to rename monkeypox virus to avoid discrimination

The World Health Organization has said it will rename monkeypox to avoid discrimination and stigmatisation as the virus continues to spread among people in an unprecedented global outbreak of the disease.

Dr Tedros Adhanom Ghebreyesus, the WHO’s director general, said the organisation was “working with partners and experts from around the world on changing the name of the monkeypox virus, its clades and the disease it causes”.

The move comes after scientists called for an “urgent” change to the name which they described as “inaccurate”, “discriminatory” and “stigmatising” in a report released last week. An announcement on the new name would be made “as soon as possible”, said Tedros.

Why the monkeypox outbreak is mostly affecting men who have sex with men

A global outbreak of monkeypox across non-endemic regions including Europe and North America was confirmed in May 2022. The current outbreak has shown distinct epidemiological features compared with past outbreaks in non-endemic settings, most notably its observed rapid growth and predominant spread among men who have sex with men (MSM). We use a branching process transmission model fitted to empirical sexual partnership data in the UK to show that the heavy-tailed nature of the sexual partnership degree distribution, where a small fraction of individuals have disproportionately large numbers of partners, can explain the sustained growth of monkeypox cases among the MSM population despite the absence of such patterns of spread in past outbreaks. We also suggest that the basic reproduction number (R0) for monkeypox over the MSM sexual contact network may be substantially greater than 1 for a plausible range of assumptions, which poses a challenge to outbreak containment efforts. Ensuring ongoing support and tailored public health messaging to facilitate prevention and early detection among MSM with a large number of sexual partners is warranted.
We need to be careful about how we draw conclusions from the data, as there is ascertainment bias going on. MSM have a better relationship with medical practitioners than heterosexual men, which could mean they are more likely to report monkeypox symptoms and get tested for the virus. “I don’t know that we’re necessarily looking enough in heterosexual social networks to make the conclusion that this is not a broader problem,” says Boghuma Titanji, a virologist at Emory University who works at a sexual health clinic.

The virus could still find other networks with similar characteristics. Daskalakis recalls a U.S. outbreak of methicillin-resistant Staphylococcus aureus in the 2000s that started in the MSM community but later spread in gyms, among athletes, and in prisons. Monkeypox could also start to spread among sex workers and their clients, Tuite says.

imo when a person associates "monkeypox" with humans in calling it "discriminatory", it says more about the person doing that than the name "monkeypox" does about anyone else.
 
It's not about that, it's about the strain designations. From the article:
The WHO currently refers to two types of monkeypox, namely the “west African” and the “Congo basin (central African)” clades. This year’s outbreak is driven by the former type, which is substantially less dangerous than the latter.

“In the context of the current global outbreak, continued reference to, and nomenclature of this virus being African is not only inaccurate but is also discriminatory and stigmatising,” the experts write. They go on to criticise the use of photos of African patients with pox lesions in coverage of the outbreak in the global north.

That goes without saying; when the tourist season begins, covid magically disappears.
Maybe there could be a 12-month tourist season, instead of vaccines.
