Blockades are my bet. Check the water tiles around your cities. Are they just slightly darkened? This happens to me a lot and I have a hard time figuring out why because it is so easy to miss. Send out your planes and ships (preferably in that order) to deal with any offending blockaders and I bet you will solve your problem.
