Does it mean they don't actually like me enough (must be pleased or friendly), or does it mean they like the other civ more than they like me (if so how can I damage that relationship) or does it mean the other guy is too powerful for them to sacrifice themselves?

Well, potentially all those can come into play but def the first two points. It can vary by AI leader. Each leader is coded a different way and their attitude toward you and other civs can affect their willingness to go to war on your request. The following strat article is very helpful:

If they liked the other civ too much the message is usually "we won't betray our close friends". Both of course it could be both in which case I'm not sure what message comes out in priority . There is also "we would have nothing to gain", or "we fear their military might".
