Well, I had the same problem when I signed up for the "demogame" a few months back.
Basically, a whole bunch of people decide on what civilization to play and what leader, and the games specifications like map size etc.
Then we form our government (its in the CoL and Constitution thread.)
Someone plays the game save ("the save") every few days as instructed by the rest of us.
The people who give the instructions hold government positions which each have defined powers, and are elected once a month.
Citizens who do not hold government positons influence the decisions made my government officials.
Also the designated player is the one who plays the save, and is elected as well.
And the rest is history...
Wish I had time to elaborate, but my pc is broken and I'm using a friend's right now.
