Carthage doesn't grow at all...whip it, add population using Worldbuilder, nothing makes it grow. Really weird.

Actually its not weird - since you activated the "Avoid Growth" Button the city is doing what you are telling it to do

This does not matter - Avoid Growth works manually as well - so you can max production/commerce whatever without thinking about the growth stuff, I don't think its good, but it works as intended
