This is a fairly regularly reported problem. Civ 4 seems to randomly delete the .ini file and start a new one. You can try write protecting the file or just keep a copy so that you can restore it when it happens.

As s.bernbam says the 3.19 patch of BTS removes the need for a disc. You will have to download and install the patch manually however as the in-game patching doesn't work for that patch. The patch is available in the downloads section on this site.
You will have got a new steam code with your DVD. If you do not activate your new code steam will only recognise your old code, the vanilla one. You need to activate your new code on steam which will then download the expansions.
To activate your code in steam select Games>Activate a...
QManNL, did you let it install in the default location? The reason the process in this thread suggests installing elsewhere is due to problems with mods (its a permissions thing with Vista/7/8). If you have installed to the default location it may be worth re-installing somewhere else.
If you have a problem with an advert try and get a screenshot of the banner ads at the top and bottom of the page. This will help the mods identify the problematic advert and get it removed.

In theory, you are correct but you shouldn't have to install any patches except the final patch for BTS as the expansions should fully patch the previous installation. In practice we have seen many occasions over the years since Civ 4 were released where the game will not install properly...
For anyone with the steam version, 2k have put the pre steamworks version as a beta download to restore DirectIP. Hopefully it will remain there at least long enough to finish current games.
