I'm okay with Egypt being the former Roman Colony, and now brother in Populism.
I imagined Venice being an independent country for a while, before being eaten by the Republic of Rome during the Liberal Revolutions (and thus giving Rome the Adriatic and Greece). I think of Rome being the equivalent of Napoleonic France, except that it convinces another power to help it out and support it before realising that Rome is a radical revolutionary republic out for blood and thus gets away with huge chunks of territory. I will do a full history of the Italian Peninsula at some point, and submit it for review/alteration based on choices. The antagonist country doesn't necessarily have to be massively involved territorially in Italy - just massively involved in terms of dynastic or political interests (see France in the Italian Wars of the 15th-16th centuries).
Also, I like Bohemia being a powerful Empire (maybe stretching from the Baltic to Rome?) as well as Great Old Bulgaria being a thing.
ED: I haven't defined Populism properly yet. It is a radical left-wing revolutionary collectivist of some kind ideology, so it fits in a Comintern.
