I just signed up for an account on these forums just so that I could pass on this information, so I hope it's helpful to other people.
Shortly after buying Civ 4, I was able to play the first part of a multiplayer game for about two hours without any real problems. For weeks after that, I was almost never able to play a multiplayer game without about an hour of my connection frequently dropping every minute or more. Often times when my game connection would drop, I would check the status of my router, my cable modem, and my Vonage box and see that I couldn't even view the information to my cable modem or Vonage box. Until about a week ago, I had Vonage, and I had a sneaking suspicion that it was Vonage that was causing the problems.
Sure enough... after disconnecting Vonage, I haven't had a single problem. Every multiplayer game runs great, and I haven't had a single problem. My frequent disconnects and "waiting for other players" messages are all gone.
I'm not suggesting that Vonage has a problem for everyone, but if you're having problems with staying connected in a multiplayer game and you have Vonage, you may want to look into this.
