How do probe teams work?

It seems that the first probe action on a base usually hasa 100% chance of success, but after that i get a message saying that the base has been equipped with a new high security system (this is way before anyone can build the covert ops center).

Also, when you first infiltrate a faction's datalinks, is there any way to go back and review that info again? Like what SE choices they had at the time.

Are you referring to the specific probe action Procure Research Data? If so, it appears that you're correct that the first attempt to steal a tech at any specific base succeeds. Subsequent attempts at the same base have a low probability of succeeding.

If you're asking whether you can review an infiltrated faction's SE choices at some point in the past, I don't think that's possible. You can review their current SE choices by selecting the Commlink button, right-clicking on the faction in question and then selecting Faction Profile.
