One of the things missing is the possibility to import food from other cities. As things currently stand it is impossible to have a city that would produce something (e.g. shields / gold) without having to rely to some food production as well (city-states assist, but in a more general manner). This makes the creation of highly specialized cities very difficult (e.g. city specialized to shield production, food production etc).
Modern societies have indeed cities that just import their food (think Las Vegas in the middle of the desert, New York etc).. A similar implementation would be nice in CivV (e.g. create a city on the hills, mining only and specialize in shield production).
This could be implemented as follows:
1. Caravan goes from city A (Exporter) to city B (Importer).
2. Route is opened for say 4 wheats.
3. Some food is lost on the road (e.g. of 4 wheats exported only 2 arrive in the Importer) due to spoiling etc. Evolution of refrigeration, roads etc should reduce the amount of spoilage.
4. Exported wheat is deducted from Exporters wheat and added (to the extent it remains) to the Importer's wheat.
5. This could also have some recurring cost per round.
