No civ switching in-game. You must leave and re-enter as the new civ.
No option to refuse city flips.
No world congresses.

Possible change:

Plague is still causing issues and may or may not be removed.

No, it's a mod. It was previously singleplayer only, but it looks like it's getting multiplayer. The dev is doing a beta test and asking people to join in.
