You should've opened the file since your guess is 100% wrong.
I double checked just to make sure and none of his worst health cities had poisoned water supply. The health problem is all homemade.
EDIT/PS: Same with the unhappiness.
The + or - is the sorting order (ascending or descending) and when you click on a leader it will sort the other leaders according to the diplomatic relations between them (in either ascending or descending order as mentioned above).
And after having a look at the save, I guess that's the reason. I assume your cities were at +/- 0 Health before you started building factories, etc. and now they're unhealthy for the obvious reasons.
Possible dirty workaround:
CMIIW, but I think you can mount to A:/B: if you don't need those letters for floppy drives (and it's been a damn long time since the last time I've seen a PC with two floppy drives ;)).

I had the problem. It said (after patching) that the game version is 300 in Main Menu, but wouldn't let me patch again with the same error message as you.
I "fixed" it by completely uninstalling BtS, deleting the BtS folder (didn't get deleted on uninstall) and installing BtS and the patch again.
How would a patch on release be an anti-piracy measure of any sort? Conspiracy theory much?
Also, a patch like that (Sync between US/EU versions?) wouldn't bother me at all if I were to pirate BtS, anyway.
I'm pretty sure that it's just the old game of developer vs. publisher where publisher...
