dx12 windows 1607 exit crash

i have had an issue 3 times now where upon saving game, then select exit to desktop, the game does not close properly. the mouse pointer moves, but doesnt revert to the standard win 10 one. the menu screen stays as if it was a background. you can alt tab but while you can select other programs...
