How do I know if a jungle or a swamp terrain will become a grassland or a bonus grassland?
Because if it's a grassland, I'd rather turn it into a forest and then make a plains out of it.

There is a pattern for bonus grassland, I think you can see it if you create all grassland map with Terraform. But wouldn't you have to turn jungle/swamp into grassland on your way to create plains anyway?

You turn swamps into forests and then forests into plains.

Like hannurabi said you basically need to observe the zigzag pattern of grassland tiles. More directly, you could fast irrigate it then reload.
