MyDocuments/MyGames/ there should be a Civ 5 folder with a MOD folder inside. All mods go there.
If it's a .rar, then extract it there. If it's a .civ5mod file...well, it's also a rar, so you can also extract it there (or let the game do it). After that you just enable it in the mods menu from the game.
