I think it's not related to settlers as this phenomenon also happens in the sea/ocean tiles. Moreover it's not related to modpacks as I never install them.

The best explanation is "BUG".......

Is there a city established in that tile? No, but there is a city nearby.

This reminds me of a Civ2 bug I used to have occasionally: I'd see some or all of the enemy starting position tiles if I'd changed my civ for a new game.
