Rather interesting, I suspect the rivers and volcanoes having the same problem might be related. Took a fairly big pause some time ago with the game but running into a similar issue with rivers sounds familiar.
I do run a fair few mods that mess with map scripts if that is indeed the issue like...
So this has happened to me a few times in games thus far, I discover a volcano but it receives no name at all. As far as I can tell they basically seem to remain dormant all game from the few times I've encountered them.

With the introduction of GS I've noticed a large discrepancy in the amount of effort put in to find volcano names for nations. For example for Sweden they dug up the names of some eroded cretaceous remnants of volcanoes whilst nations that actual have multiple volcanoes only have 1 or 2 names...
Realistic or not, settling next to Vesuvius you generally expect some fireworks at least once during your playthrough given that natural wonders are permanently active and never go dormant.
One thing I've noticed though that really annoys me is the discrepancy in effort spent to research...
