Nice point about the gifted cities...I'd like to add to that one. If you attack a civ who expanded their second city to a spot near you that you covet...declare war and head there but give him time to mobilize his units. Then kills his units near the city and bombard the city...he will offer it...
I find the only way I can wage war now is to start with tradition and expand fairly early to 3 cities. This means with those three cities I'm saving 6g/turn on monuments plus have a faster growing cap for more unit spam plus take the happiness policy and connect my stuff fast. Doing it this way...

Yeah there are times where I don't even have a cs or civ within trading distance of my Capitol. I'm forced to hard build a settler quite often...I also seem to have an inordinate amount of starting locations that are super heavy on one resource type to the point of me being unable to unload...
