If you won most your games in Civ4 by Domination or Conquest, you will probably love Civ5. Warfare wise, Civ5 is vastly improved over Civ4 though I do think the its somewhat easier. I routinely played Civ4 on King, but play Civ5 on Emporer.

I miss the 'Regenerate Map' button and the scoring system in Civ4 seemed to make more sense. I played hundreds upon hundreds of Civ4 games simply to improve my high scores.
Otherwise, I think the Civ5 is an improvement over Civ4.
Of the items you mention, I have only seen the Graphic Overlay issue and typically only once per game. Extremely minor item to me.

I think two simple changes would fix most the issues...
(1) Only allow one interaction with a City-State per turn.
(2) Get rid of the 'Blocked Peace' state and allow interaction at double or triple the normal price.
Truthfully, I am fine with an Economic Victory similar to the cold war...
Shouldn't having a small civilization have some disadvantages? I think partially tying Research to population is a good mechanic for that. Large spawling empires certainly have a number of disadvantages.

I believe bike is correct. If the defender is at 1 HP, the attacker will not retreat regardless of quality. Thus a 100% retreat-unit would have the following results: Win, Retreat, or Lose to a 1-hp defender.
Isn't Rush Building more beneficial than Upgrading, so shouldnt it cost more?
Rush Building (assuming doing so with gold) gives you quite a few advantages. It frees up the city to produce something else and it gives you a new unit. Whereas upgrading converts an old unit to a new one. So...
I "Believe" you get the tech that has the lowest research cost. So if other civilizations have some of the first tier techs, that may make them the lowest cost.
