I recall the few occassions I'd encounter a land where barbarians took hold. Usually it happens very early in the game, but once again very rarely. They have to take over a city of a very weak civilization and be able to defend it. Aside from once or twice where it happened naturally, I've made situations like this. When Knights where still the peak of offensive technology I'd let barbarians who landed from the sea take over freshly conquered cities. Now remember, there are two types of barbarians. Those made from insurgencies/rebellions that just appear and those that come from ships. The ones from ships (which usually stop appearing later in the game) are the ones that can take over cities. The others just destroy them.

When they take a city, they don't build buildings and you can't establish an embassy or talk to them. The fields are blank (sort of like after you already have an embassy and try to make one).

When the Barbarians take over a city they just keep generating barbarians. Hordes of them. In games that I wanted to remain isolated, but had to destroy another civilization to stop raids I have just abandoned the cities until Barbarians took them. Eventually, the entire continent is overwhelmed and flooded with them. Enough so that it is hard to find a beachhead sometimes without making one. Furthermore, even if you drop a transport of armors you still do lose a few after they swarm you.

Remember, often chariots/knights have a good chance on grasslands for winning against an armor when on the offensive. If you are in the stoneage still and some armors land I'd let them take one city and wait outside or near with a force of knights/chariots. Storm them and take back the city and claim the Automobile as technology. I've even given up their cities (usually the ones I know I can't hold) and let them come back after I sold the city walls and came back the next turn to take it only for the techology. Often, it kills the city down to 1 when you do it and you end up destroying it. But, it was an enemy city anyway. ;p
 
Once saw a little continent of barbs. 4 red cities on an island spawning units. Loads of fun to run over them.
 
I love letting Barbarians have their own little island. I'd frequently find a Civ on a small(ish) island and capture their biggest city, and raze the rest, then leave it defenseless so a barb would take it. It was always fun to me to see the AI of the barbs in action, even though it's MO seems to just be "spawn lots of units and swarm."

Biggest barb city I can recall seeing though was size 6, and for some reason it seems like the barbs only ever left one defender in a city, so they were easy conquers.
 
Send a nuke at 'em. Boy will they be surprised (for about one tenth of a second).
