I've been playing CIV 1 for years. My biggest city is at level 42... Anyone ever had a bigger city? And how did you do that?

42 is the limit if your city is working 21 grassland squares with railroad and irrigation on all of them, including RR on the city square (21 x 4 = 84 food; 82 / 2 = 42 pop).
The theoretical limit without hacking is 46, which can only be reached if 4 of the 21 squares are oases, which give 6 food when fully developed.
The top limit is achievable with hex editing. Then you can have a city on all oasis squares produce 21 x 6 = 126 food, enough to sustain a population of 63.
