I finally found how to make Civ4 WorldBuilder place rivers where I want. The following tips all assume you are in "river placement mode".

---------------

Rivers only exist on the bottom and right sides of the tiles.

Place a river by left clicking the starting tile, then left clicking again on an adjacent tile that you want the river to flow into.

The green square with the river symbol indicates the starting tile. A pale white overlay shows the adjacent tiles that you can click to form the river.

The second tile will become the new starting tile with possession of the river symbol.

The placement tool tries to go straight so for a horizontal river, it will fill in the bottom of both tiles. For a vertical river it will fill in the right of both tiles.

It always places at least two sections of river so you often have to erase unwanted sections.

If the destination tile is diagonal it will place two, three or four sections, always on the bottom and right of the tiles and always as straight as it can.

Left click a non adjacent tile to start placing a new stretch of river.

Right click to stop placing rivers. You need to do this when you want to erase or go back to the old starting tile and often when you want to change the direction of the river.

To erase both rivers on a tile (bottom and right sides) right click the tile. You cannot select just one river. This means you sometimes have to erase and replace sections you wanted.

You can hold the right button and sweep the mouse to erase a lot of river.

You can hold the left button and sweep the mouse to create a long river. Expect the unexpected.

Reference: The only information I could find anywhere on the web was two sentences in "Modding Guide for Civilization IV" by Dale Kent. It is available as Civilization IV World Builder Manual

"Rivers can be tricky to work out correctly as you need to specify the starting square and then click in the tile the river flows into. Don’t worry if you make a few mistakes while working river placement out as you can right-click on any tile with a river in it to delete the river from that tile."

Thanks Dale. This was the snippet that allowed me to figure it out.
