I was playing med 2 today and briefly left to get something to eat, I returned to find the monitor had no signal and the sound was looping (obviously a crash). I rebooted and the card seemed to boot fine also but there was no signal on the screen. There is one led the red LED that flashes once (which is normal) but nothing other than that.

Check all your cable connections. But it's possible the card just burned out. try an alternate card and see of the rest of the PC works.

Install a temperature monitoring program, such as RivaTuner ( I think it works for AMD cards as well as nVidia) and watch your GPU temps, that is, if you can get it to boot again.

Also, open up the case, check all connections, and see if there are any burn marks on the parts of the card that arent covered by the cooling shroud. Try it in another pc if you can, and if its dead there too, then the card should be RMA'd. If it works there, check your motherboard, you may have had the PCIe slot fail, or the whole motherboard fail. Check if its booting ( beep codes, code display on the board itself, LED's, etc)

I took the whole thing to the repair shop yesterday (they have free diagnostics) is suspected it was a card failure, and indeed it was. Conveniently, I got the card from the store so they replaced it. I wonder what caused it to burn out though. Was my power supply not keeping up? Or was the card just a lemon? (I doubt it was cooling, my case is a full tower with 6 fans ).

Well, even still you want to check the card temps. A 6-fan setup is nothing if it has a bad airflow layout that creates pockets of hot air around the GPU.
My case has 7 fans but I cant turn all of them on at the same time as that just makes things worse, they completely nullify each others airflow.
