I got this idea from Battle Realms:

I don't know if this has been suggested before, but maybe in Civ4, they should implement a system where injured units suffer an attack/defense/movement penalty to increase the realism. What do you guys think?

they already suffer a combat penalty. it's called less life :p

I agree with ybbor. The health bar indicates the general health and morale of the unit. Decreasing a/d/m would be akin to infantry, when injured to 50% HP, wielding arquebuses and wearing only a light layer of clothing.

They tried that in Civ II as well. I'm not sure why it left. I think it had something to do with difficulty in knowing how many squares you had left in your movement. I could be for this if this issue was resolved. (It does make sense to have health affect movement.)

It was also in SMAC, your attack would decrease depending on how many spaces you moved before attacking
I kinda liked that. It made the combat more strategic, and a little more realistic. A forced march can take a lot out of you.
