From what I can discern...

Name: This is your name. (ie. Bismark)
Formal: The name of your civ (ie. Germany)
Noun: The name of your people (ie. Germans)
Adjective: The name describing your cities, units, etc. (ie. German)
Title: Your title (ie. Chancellor)

Of course, you can get creative. My favorite so far is MP Moderator Chieftess of the Spammers.
