I'm not sure how to express myself clearly about this.. here it goes:

So lets say you have a few cities scattered around within a relatively close distance..
And then somewhere in the middle you make a farm.. Which city will benefit ?

Sometimes when I play, one of my cities has 50 (hammer) and the one 4 tiles away has like.. 7.. but they both have 10+ population

In such a situation, either city can work it, but only one can work it at once. That would account for the discrepancies in your cities.

Easy. You open the city screens. Open the citizen tile thingie (+ symbol, top tight). The cities that can't work the tile show that tile with a black button with two swirly arrows, showing another city is working it. Find the one that's working it, click to to take the citizen off the land, and reassign in your desired city.

It is possible though that your culture can go out further than your city can work the land, especially if your cities are further apart. While you can build an improvement, your cities may not be able to take advantage of it.

That is true. You can only work tiles in a 3 tile radius, but your city borders expand out to a 5 tile radius.

You can develope the resources (gold, pearls, oil, spices, etc..) even if your city "workers" cannot work them as long as they are in your cultural borders. You will get that resource to use for city demands or for trading.
