Making maps from scratch

I must be stupid since I've tried following different guides and such but I'm becoming frustrated.

I want to make a map from scratch. I want to make the landmasses, place the resources and starting positions, and I want the players to be able to choose their civilization and the AI to be random.

How do I do this?
