I've seen this happen twice in multiplayer using Gamespy.
I'm not sure what causes the problem, and as it happens before the game starts - I don't think a saved game file would help.
Everyone has clicked on "ready", however when the host clicks on "launch" the game does not launch. There is no error message.
Solution: change the map size (ex. increase it from small to standard), and then change it back.
