I've found a way around the issues I previously raised in my "Iraq or belgium" thread but I'm pretty certain that my find highlights another (vanilla Civ4) issue... razing to the ground entire cities, often of (presumably) many millions of people allows me to eliminate all possible revolution/insurgency issues.
However in the real word & esp. the modern world putting many millions of people to the sword is (thankfully) actually quite rare... problem?
I have a suggestion.
The worst crimes in history should be punished at a rate dependent upon your (or there) media level.
Cultures pre-writing should be able to annihilate cities exactly as now but as new key technologies are learnt the foreign relationship penalty for slaughtering cities/ethnic cleansing should be increased (currently if I was to invade the UK in 2008 as the TRFSEAS* & kill every man, woman & child in London that would cause an additional -1 relationship penalty with the UK... exactly that which I would receive if if I refused to grant them the technology of, lets say, Fascism).
In addition I really think there should be at least some revolution/happiness penalties for cultures using any of the 'goody-two-shoe' civics (free speech, universal suffrage, free religion etc) who go around slaughtering entire continents.

Further in regard to insurgencies; even after reading the cool “insurgencies cause random events that destroy farms or roads” proposals I've seen, I'd just like to say that sometimes the most painful thing about war is the continuous 'war weariness'... maybe THAT'S how a small power defeats or, more correctly, survives a larger one?
Also war weariness should probably make revolutions more likely (1917 Russia would be the prime, but certainly not the only, example).
Combined with my previous thread about “Iraq or Belgium” I see that cultures either do not fear 'nasty powers' enough (i.e. Belgium would launch a vicious insurgency against Hitlers Germany) or 'nice nations' can get away with mass, indiscriminate slaughter too easily. The mod encourages a game of extremes (either avoid war entirely or slaughter everyone)... my inherent fatalism is probably highlighted by the fact that I'm not entirely sure even if the mod is actually 'wrong' or not in this regard.

Further cool things I've done in your mod
Gifted overtime several cities to a neighbouring civ just to avoid revolutions (Once I discovered the required tech the grateful buggars became my vassal)... this was on a continents racked with continuous war & revolutions. Our continent became a hodge-podge of nations with totally illogical borders (aka medieval/renaissance Europe) until i eventually 'conquered' it all through a series of vassalage agreements*. Our Continent was so bloody that we didn't learn 'assembly line' til 2017... yet I then dominated the world by 2043.
* Even then mine was a rather odd looking kinda chicane-shaped country.
